Drug and Alcohol Assertive Outreach
As part of the HORIZON Project, our Assertive Outreach Service helps re-engage individuals who have dropped out of treatment and supports those most at risk, including chaotic drug users and the homeless.
Our Approach:
- Re-establish contact with clients who have a history of missing appointments or struggling with treatment.
- Motivational interviewing techniques to encourage re-engagement.
- Encourage dependent drug and alcohol users to access treatment.
- Collaborate with hospitals, ensuring patients admitted for drug- or alcohol-related issues receive continued care after discharge.
- Support street drinkers by providing outreach and linking them to treatment services.
